Wraparound for Complex Needs

A Wraparound facilitator helps families with children or youth with complex and multiple needs to identify their unique strengths and needs, develop a personalized plan to address them and build a team of both informal and formal supports to assist them. The facilitator may access community resources and donations of goods and services for families. A Community Team guides and supports the Wraparound process and is committed to developing partnerships between families and the local community. The Team is made up of parents, service providers, business leaders, faith communities, and volunteers.
