Halton Intensive Program
The Model of Service
This program is an intensive home based model servicing children between the ages of 6-11 (with flexibility if
clinically warranted) and their families, with mental health issues as their primary area of concern. These children
may also have complex needs and include children with a developmental disability, in the mild range of cognitive
impairment, where mental health has become the primary area of concern. This program is designed to provide up
to 6 months of intensive service, with up to 3 months of follow-up support.
The model of service will incorporate elements from models such as Multisystemic Therapy, Collaborative Problem
Solving, and Cognitive Behavioural Therapy, and will be identified in the child and family’s individual treatment
plan. This model also incorporates the strengths of Wraparound for service coordination and collaboration. The
principle elements include: strength-based, family-centred, community-based and culturally competent.
The model also provides for access to a Day Treatment component for those children having difficulty in school
settings. The Day Treatment will be available only to those children within the intensive program, and will be
individualized based upon client need.
The Intensive Team includes a Manager, Social Worker Coordinator, 2 Full Time Intensive In-home Child and
Family Workers, Classroom Teacher, 2 Classroom CYW’s. This program also includes Psychiatric and Psychological
consultation for the intensive program, within a Multidisciplinary Team approach.
Referral Process
The entry point to the Halton Intensive Program will be through Service Resolution after a Service Coordination
meeting has taken place between the family and all services working with the family. Each referral to this program
will be presented at Connections Halton. Cases will be prioritized according to need.
Once assigned to the Halton Intensive Service, the Woodview team will work collaboratively with all sectors to
develop a system of care and family supports to provide the intensive in home services individualized to each
family’s needs and strengths
